Holystik Care delivers non-clinical Home and Community-Based Services (HCBS) designed to support independence, dignity, and quality of life for older adults and individuals with disabilities. Our services are structured to align with Illinois state programs and regulatory frameworks, ensuring compliance, continuity, and compassionate support across multiple pathways of care.

Community Care Program (CCP)

Holystik Care is an approved provider under the Illinois Department on Aging’s Community Care Program (CCP). CCP is a Medicaid-funded Home and Community-Based Services program designed to help eligible older adults remain safely in their homes and communities.

Under CCP, Holystik Care delivers the full scope of authorized non-clinical home services, strictly in accordance with IDoA-approved care plans and in coordination with assigned Care Coordination Units (CCUs).

CCP Non-Clinical Services May Include:

Personal Care Assistance(Non-Medical)

Support with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting assistance, mobility support, and safe transfers. Services are provided with respect for dignity, privacy, and independence.

Homemaker Services & Household Support

Assistance with light housekeeping, laundry, linen changes, dishwashing, trash removal, and basic home organization to maintain a safe and functional living environment.

Meal Preparation & Nutrition Support

Support with planning and preparing meals based on individual preferences, cultural considerations, and care plan guidance, ensuring meals are safe, accessible, and nutritionally appropriate.

Companionship & Safety Supervision

Ongoing presence to reduce isolation, promote emotional well-being, and provide safety oversight within the home environment.

Errands & Essential Community Support

Assistance with essential errands such as grocery shopping, pharmacy pickups, and other approved community activities that support independent living.

Respite Support for Family Caregivers

Short-term non-clinical support that provides relief to family caregivers while ensuring continuity of care for the older adult.

CCP Eligibility & Qualifying Criteria

To qualify for services under the Community Care Program (CCP), individuals must generally meet specific age, residency, financial, and care-based requirements.

Criteria 01

Age Requirement

Individuals must be 60 years of age or older to be eligible for the program.

Criteria 02

Residency

Applicants must be a documented resident of the State of Illinois.

Criteria 03

Financial Eligibility

Individuals must meet specific financial eligibility requirements as defined under Medicaid guidelines.

Criteria 04

Care Assessment

Must be assessed by a Care Coordination Unit (CCU) and determined to be at risk of nursing facility placement.

Criteria 05

Approved Care Plan

Must have an Approved IDoA Care Plan specifically authorizing the necessary services.

MCO Integration

MCO Enrollment

Individuals enrolled in a Medicaid MCO and authorized for CCP are eligible for Holystik Care services, subject to coordination between the MCO, IDoA, and the CCU.

How CCP Works with MCOs

Illinois delivers many Medicaid-funded services through Managed Care Organizations (MCOs). For those enrolled, CCP services remain care-plan driven and IDoA-governed.

Step 1

Assessment & Planning

The individual is assessed by the assigned Care Coordination Unit (CCU), which develops and maintains the CCP care plan in alignment with IDoA guidelines.

Step 2

MCO Coordination

The care plan and service authorizations are shared and coordinated with the MCO’s care management team to ensure continuity and accountability.

Step 3

Service Authorization

Services are authorized based on the approved CCP care plan, with oversight from IDoA and coordination with the MCO as applicable.

Step 4

Service Delivery

Holystik Care delivers authorized non-clinical services, maintaining communication with the CCU and, when required, the MCO.

Step 5

Ongoing Monitoring

Needs are reviewed through reassessments, with updates coordinated among IDoA, the CCU, and the MCO to ensure services remain appropriate and compliant.

Non-Clinical Home Services (IDPH)

Holystik Care is licensed by the Illinois Department of Public Health (IDPH) to provide non-clinical home services for individuals in Cook, DuPage, and Will Counties who seek private-pay or alternative funding options.

How IDPH Services Differ from CCP

No Medicaid Ties

Services are not restricted by Medicaid eligibility, allowing for broader accessibility.

Direct Planning

No CCU assessment or IDoA care plan is required; we work directly with you and your family.

Flexible Scheduling

Service frequency and duration are determined collaboratively to suit your specific lifestyle and needs.

Ideal for Support

Perfect for individuals seeking flexibility, supplemental support, or interim care during eligibility changes.

Home Services Program (HSP)

Administered by IDHS through the Division of Rehabilitation Services (DORS), this program supports individuals with disabilities by promoting autonomy and community integration.

HSP Focus & Services

Participant Independence

Supports individuals with physical disabilities and encourages choice as an alternative to institutional care.

Personal & Household Care

Includes personal care assistance, homemaker services, and support for daily routines and household tasks.
