How Referrals and Service Coordination Work
Whether a referral comes from a Care Coordination Unit, case manager, MCO, hospital partner, or family member, our approach remains consistent:

2. Program Alignment – Clarifying eligibility, funding pathways, and authorized services
3. Care Coordination – Working with CCUs, case managers, and families to support approved care plans
4. Service Delivery – Providing consistent, non-clinical support within defined program scope
5. Ongoing Communication – Maintaining transparency as needs evolve
This process helps reduce delays, confusion, and duplication—allowing everyone involved to focus on what matters most: the individual.
